Escalation Form for Authorized Assister
The Assistance Network Escalation Form should be utilized when resolution is not met when contacting the Connect for Health Colorado Customer Service Center. All sections of the form should be completed before submitting. Please keep this form free of customer Personal Identifiable Information (PII). Once the form is submitted, the Assistance Network Team will connect you with an Escalation Team Member, who will contact you via email or phone, to work with you to meet your customer’s desired outcome.
